Athletics School Physicals Information

School physicals will take place at the nurse’s office on the building dates listed in the attached PDF. Please make sure to register on Final Forms https://sachem-ny.finalforms.com/ prior to your child taking a physical if they have not done so already. All medical forms that may be needed such as NYS physical forms and other medical forms are located at https://sachem.edu/departments/athletics

Building nurses will be available only on the following dates throughout the summer: 7:30am-2:00pm
HS: August 1, 3, 15, 18, 21, 22
MS: August 21, 22, 23, 29, 30

Fall Start Dates
8/21: High School
9/7: Middle School
