An annual physical exam is required for every seventh grade student by the school physician.  A student is encouraged, however, to have his/her family doctor provide this function of the health program.  In addition, aural, visual and scoliosis checkups are given through the Nurse's Office (Health Office).

A student must have written permission from your parent/guardian and a school physical examination before they can participate in any athletic practice.  The Sachem School District provides physical examinations each year for those students involved in sports and extra curricular activities.  These examinations are announced well in advance, so it is up to the individual student to process the required paperwork in a timely fashion.  Students are not permitted to participate in inter-scholastic sports without this physical examination.  You may have your family doctor provide this service if so desired.  The proper paperwork must be forwarded to the school nurse in timely fashion for verification.

A student who becomes ill during the day should report to his/her teacher who will fill out a pass to the nurse for that student.  Once at the Health Office the nurse will determine the proper course of action.  If the student is released, it will only be done with the parent's approval. 

MEDICATION

The school nurse, upon parent request, must assist any student who is required to take medication during the regular school day (medication prescribed by a physician).  Such medications shall be kept in the Health Office.  Students should not keep medication on their person or in their lockers.  Other health services are limited to first aid, short rest periods, and some TLC.

SPORT PHYSICAL INFORMATION

All students must have a sport physical done before the start of their first sport. This physical may be done by their private physician or by the school doctor. The Sachem physical exam form must be used. Permission and history information must be signed by a parent/guardian. Athletic physicals are valid for one calendar year from the date they are administered. The doctor must sign and date this form.

If a student has already had and passed a sport physical for a previous sport this school year, he/she need not have another physical, however, the student's parent must complete the medical questionnaire form (green card) and return it to the nurse as soon as possible before the new sport begins. Announcements will be made before the start of each new sport season.

Girls: Basketball

Boys: Volleyball and Wrestling

will be given during school hours on Thursday, January 19, 2012. In order to have the physical at school, completed forms must be returned to the nurse prior to January 18, 2012. No forms for the school doctor will be accepted after January 18, 2012. Coaches can not accept physical forms.

Failure to turn in the necessary paperwork at the designated time will result in a delay in participation for your sport.

Coaches will announce tryout dates at meetings.

*REMINDER*

To all parents of incoming 6th graders:

Students must have proof of varicella (chicken pox) disease OR immunization before they can enter the 6th grade. This state law became effective in the 2005-2006 school year.

All 6th graders must have proof of Tdap vaccine. This law becomes effective September 1, 2007.

NYS mandates for school screening have changed: Vision, hearing, color perception are done on all 7th graders (and new entrants in all grades).

Scoliosis screenings are done in 6th, 7th and 8th grade.

All 7th graders are required to send in proof of a physical exam, Body Mass Index (B.M.I.) and dental certificate to the Nurse's office a.s.a.p.
