Suffolk County - Pertussis Notification
Sports Physicals
The School Doctor will be here on February 29th at 9:00 a.m for all those students interested in playing a spring sport. Please have all green cards and sports physical paperwork handed in to the Nurse's office by February 27th, in order to be given an appointment to see the School Doctor. Please provide at least 24 hours to process all paperwork. Failure to submit all necessary paperwork to the Nurse's office by the designated date will delay participation in your sport.
As always, any questions regarding your sports physical status should be directed to the Nurse's Office. Please stop down if you require clarification.
