Required Documents for Change of Address Within the Sachem School District
Proof of Residency:
Two proofs are required:
Homeowner – Copy of either the deed, mortgage statement, tax bill or closing papers
Renter in Apartment Complex – Original signed lease
Renter in private home – An Affidavit of Residence signed and notarized by both the tenant and the homeowner. With the affidavit, the homeowner must also submit a mortgage statement, deed or tax bill for the property and the renter must submit an original lease or rent receipt
Plus
Utility bill – i.e. telephone, LIPA, cable water, etc. or insurance bill can be accepted as the 2nd proof. A driver ’s license in not acceptable.
